- All forms and applications must be submitted electronically. Those forms that are still submitted via PDF can be downloaded, completed, scanned, and uploaded on the Forms page. Updates are made there as forms move over to their new electronic format. The new format will give automatic confirmation of receipt and updates as application materials are received (coming soon).
- Note: License Renewals are can only be submitted online. Paper renewals are not accepted. For those who are using the September 1, 2020 extension, the Verification page has been updated to reflect that in the table. You may click on the “More Information” button to print any information you may need for your workplace, insurance carriers, etc.
